Sounds like a GA-40 or GA-40T. See below for the order of the tubes:

5Y3GT Transformer 6V6 6V6 12AX7 6SQ7 5879 5879

rectifier output phase | preamp
inverter |

Tremelo Oscillator on GA-40T
(metal can)

Most of these tubes should be available from Antique Electronics. I’ve
had my GA-40T for about 20 years. Nice amp, really howls!
